The answer is to hold it whatever way works best for you, but I guess you’re asking because it’s not comfortable?
To me, it looks like the remote is sitting too low in your hand. Stretch out your thumb so it’s flat then position the remote so that the D-pad is under where the thumb is, then you won’t have to bend your thumb so much to press the A button.

You should try putting on a Wii motion plus thingy that has a built in jacket, like [this one on Amazon](https://www.amazon.com/Nintendo-MotionPlus-Motion-Remote-Jacket-black/dp/B00563OBKG) but I’m not sure if this one is a good one, it’s just a reference. I have big hands too and it helps out for every game I play.
Nah definitely not, my fingers wrap all the way around the remote and it works perfectly fine

Maybe keep your hand lower so you can push the a button if that is the problem
